But it’s important to be prepared by having advance directives. These are written, legal instructions about your preferences for medical care if you can’t make decisions for yourself. Advance directives guide your medical team if you’re terminally ill or seriously injured, in a coma, in the late stages of dementia or near the end of life. By having these documents ready and filed in the Arizona Advance Health Care Directive Registry at azsos.gov, you can get the care you want, avoid unnecessary suffering and ease the decision-making burden of your loved ones at a time of high stress or grief. Here’s a quick look at the documents: Power of attorney – A medical or health care power of attorney advance directive names a person to make decisions for you when you cannot. This can be a spouse, relative or friend. Living will – This spells out which medical treatments you would and would not want to be used to keep you alive. It also spells out your wishes for pain management and organ donation. Talk to your doctor if you have questions about possible end-of-life decisions and include them in your living will. For example: • Do you want doctors to restart your heart and do CPR? • Do you want a ventilator to breathe for you? • What about dialysis if your kidneys no longer work? • Do you want antibiotics to treat an infection if you’re near the end of life? Do not resuscitate and do not intubate orders – These are prehospital Medical Care Directives – do not resuscitate (DNR) and do not intubate (DNI) orders. After downloading and completing the forms, you can file them with the registry so they’re accessible to Arizona health care providers in case of an emergency. If they’re ever needed, Arizona health care providers can access them to determine your wishes. Finally, talk with your family, close friends and physician about the documents and the decisions you have made.
